UNDERSTANDING YOUR RIGHTS
It is your right to consent to LASIK or to refuse any treatment or procedure, at any time prior to its performance.
It is your right to be provided with information – material for a reasonable person in your position – to use in deciding whether or not to undergo the procedure, including a description of the procedure, the risks, expected benefits, alternatives, and post-operative plan.
It is your right to be provided with the information listed above in language and terms that you understand.
It is your responsibility to make sure your LASIK surgeon is aware of any questions or concerns you may have and whether everything is clear to you. If you have any concerns at all, please don’t assume; ask questions.
SIGNING THE FORM
Read the material on this site and provided for you by our office; ask all your questions until you are satisfied with the answers. When you decide to undergo laser eye surgery, you will need to sign the Patient Consent Forms, indicating that you have been advised of the nature of the your particular eye surgery procedure, its risks, benefits and alternatives, and that you are making an informed decision to undergo eye surgery. Your LASIK surgeon will sign and witness your signature. A copy will be provided to you upon request.
