HEALTH PLANS:
Seibel Vision Surgery is an in-network provider for many of health and vision insurance plans. To find out whether we are providers with your insurance, please contact us in advance of your appointment. We provide complimentary insurance billing and will help you obtain prior authorization for covered procedures which require this process. Under many circumstances, insurance companies consider LASIK and some other procedures to be non-covered services. As each case is unique, we encourage you to learn about your own plan’s offerings, and we will assist you in determining your coverage where possible.
Cataract surgery is covered by most insurance, including Medicare. To find out about what is covered by your private healthcare insurance, contact your plan provider or provide us with a copy of your insurance card and we’ll check for you.
Your out-of-pocket expenses for Medicare or private insurance will depend of the type of coverage you have, whether you have supplemental insurance and whether you have a deductible and/or co-payment on your policy.

If employees expect to incur medical expenses that won’t be reimbursed by their regular health insurance plan, a Flexible Spending Account is for them.
